Mold in a basement can absolutely affect the air quality and health of the floors above it, mainly through airflow, not through mold physically climbing the walls. Homes don't have sealed-off levels — air moves between floors constantly through stairwells, gaps around pipes and ductwork, and especially through a forced-air HVAC system, which can actively pull basement air (and any mold spores in it) up through the vents and circulate it through every room in the house.
How It Actually Travels Upward
The HVAC system is the biggest pathway. If your furnace or air handler sits in the basement, and mold is growing anywhere near it — on nearby framing, in ductwork insulation, on a damp wall — spores can get pulled into the return air and distributed through the vents into every room the system serves. Treating basement mold as a contained, basement-only problem is the most common mistake homeowners make with it. If the basement mold is anywhere near the HVAC system's intake, ductwork, or the air handler itself, remediation needs to account for the whole air system, not just the visible mold on the wall — otherwise spores already circulating through the ducts will simply reintroduce mold to a "cleaned" basement, or continue affecting air quality upstairs even after the original spot is gone.
This is also why sealing off the basement door and hoping the problem stays contained rarely works as a long-term fix. It might slow the spread, but it doesn't address the air system doing the actual distributing.
